Japan's health system lets you walk into almost any medical institution without a referral. That freedom feeds the misunderstanding that you should head straight for a big hospital. Knowing how the system works — the family doctor (かかりつけ医, kakaritsuke-i), referral letters and the #7119 emergency advice line — makes getting care faster and cheaper.

Without a referral letter you pay ¥7,000 or more extra, and waits are long. Start at a clinic and ask for a referral letter if needed.
Ambulances are free, but call-outs reached about 7.6 million in 2023 (a record), and the Fire and Disaster Management Agency flags that about half are for minor conditions. If unsure, call #7119 and ask for a judgement on urgency. If it is a real emergency, call 119 without hesitating.
A doctor who sees you over time can judge with your history, medicines and lifestyle in mind, and refers you faster. Choose one clinic that is close to home and easy to talk to.
