Facial massage and face rollers (biganrōrā) sold in Japan often promise a "smaller face" (kogao) or a "lift". Many people have felt puffiness go down and their face look fresher for a while. Whether sagging can be "lifted" is a separate question. Starting from how the skin is built, this article separates what these can and cannot do.
| Concern | Cause | With massage or a roller | Realistic measures |
|---|---|---|---|
| Puffiness | Fluid retention from salt, alcohol, lack of sleep and so on | Eased for a while (a few hours) | Cut back on salt and alcohol; raise the pillow slightly |
| Sagging | Loss of collagen in the dermis, descending fat, changes in the facial muscles | Does not lift | Sun protection, moisturising, and a dermatology or cosmetic dermatology clinic if needed |
| Dullness | Reduced blood flow, a disturbed outer skin layer, pigmentation | Blood flow gives a brief brightness | Sleep, rethinking how you wash, sun protection |
| Smaller face | Bone structure and the amount of fat | Bone structure does not change. The face looks slimmer only by the amount of puffiness lost | — |

Sagging is caused by changes inside the skin's structure and does not come back by rolling from outside. Puffiness going down and the face looking fresher for a while is a different thing from improving sagging.
The skin is thin, and strong friction causes pigmentation, fine lines and bruising. "It hurts, so it must be working" does not apply to the face.
Without something to glide on, the friction goes straight into the skin. Apply milk lotion or cream first, or use it when the skin is wet, such as in the bath.
Facial puffiness can ease for a while, but there is no firm evidence that "flushing out waste changes skin quality". This is an area where wording needs care.
Most are general goods or beauty appliances. Products approved or certified as medical devices carry a "medical device certification number" (医療機器認証番号) on the label. The most reliable way to judge the claims is to look for that label.
