What are the common misunderstandings about bad breath?

The answerThe biggest misunderstanding is "bad breath comes from the stomach." About 90% of bad breath starts in the mouth (tongue coating, gum disease, reduced saliva); the stomach is rarely the cause. The basics are cleaning the tongue, cleaning between the teeth, water and saliva, and a gum-disease check at the dentist. There is also a known condition, halitophobia (自臭症, jishūshō), in which a person worries intensely about bad breath they do not have.

Bad breath is hard to notice in yourself and awkward for others to mention. Since 2020, posts saying "I noticed my own breath behind a mask" have increased on forums abroad too. Here we look at why most causes lie in the mouth, and at the problem of worrying too much.

  • The Japan Dental Association explains that about 90% of bad breath comes from the mouth, with tongue coating and gum disease as the main causes. "It comes from the stomach" is repeatedly pointed out as a misunderstanding.
  • Morning breath (physiological bad breath) happens to everyone and is caused by reduced saliva during sleep. Brushing and a drink of water in the morning deal with it.

A toothbrush does not remove tongue coating or debris between the teeth. Tongue brush once a day; floss or an interdental brush.

It masks the smell for a while. Remove the cause first. Alcohol-based mouthwash dries the mouth and can make things worse.

Over-scrubbing damages taste and the lining. Gently, back to front, a few strokes, once a day.
